Job Description Our client in the Non-Profit/Health Care Sector is currently seeking a Placement Coordinator to join their team on a contract basis starting on a 3 month contract.

This role will be onsite in Westboro, with hours Monday-Friday 7am-3:30pm.

The successful candidate will come with previous experience working in the medical field.

Responsibilities: Collaborate with Champlain LHIN (Local Health integration Network) to maximize full occupancy and attend meetings,



as needed Review all resident applications for admission to determine appropriate level of care; assist with processing of admission and discharge papers Maintain the waitlist and internal transfer list Coordinate internal transfers including the update of documentation Process paperwork for deceased residents Provide Resident Admission Packages for Families and Nursing Ensure all Resident Health Cards are updated: renewed, cancelled or address changed Meet with families and help coordinate smooth transition for admissions including the review and revision of the Admission Agreement Complete and submit High Intensity and Lab Claims forms in a timely manner and ensures funds received Handles and ensures the protection of extremely confidential and sensitive information and material Requirements: 2 years of experience working in Healthcare 1 year of Administrative/Coordinator experience Empathetic and robust communication skills Organized and effective time management
