Join Trillium Health Partners as a Social Worker in the Early Psychosis Intervention Program. Provide critical support and treatment to adolescents facing first episode psychosis in Mississauga.

This full time Social Worker role requires a Master's degree in Social Work and at least three years of experience with young adults experiencing psychosis. You will collaborate with clients, families, and treatment teams to develop and evaluate treatment goals. Your expertise will facilitate effective case management and community coordination.

Key Responsibilities:



• Conduct clinical assessments for adolescents with psychosis • Develop and evaluate individualized treatment plans • Educate families on support strategies and resources • Collaborate with community agencies for comprehensive care • Manage cases to ensure continuity of treatment

Requirements: • M.S.W. from an accredited program • Registered Social Worker (RSW) in Ontario • Minimum three years post-graduate experience • Knowledge of the NAVIGATE model as an asset • Excellent interpersonal and assessment skills
