Key Responsibilities:
• Optimize drug therapies and improve patients’ quality of life
• Engage in patient interviews and medication history reviews
• Participate in quality improvement and report adverse events




• Use critical thinking for order entry and verification
• Teach and mentor pharmacy staff as required
Requirements:
• Bachelor in Pharmacy; Pharm D an advantage
• Valid registration with the Ontario College of Pharmacists
• Hospital experience preferred with advanced certifications
• Strong prioritization and independent work skills
• Effective communication skills for collaboration
