Key Responsibilities:
• Educate Indigenous patients on dialysis options
• Remove barriers to home dialysis
• Collaborate with health teams for effective transitions
• Travel to communities for support and education
• Promote patient voice and empowerment in health decisions
Requirements:
• Three years’ experience in patient care
• Bachelor’s degree in Social Work or Nursing
• Valid driver’s license and access to a vehicle
• Understanding of Indigenous health challenges
• Proven ability to work within Indigenous communities
