Join Community Outreach Services as a Community Support Counsellor, providing essential mental health support in East Toronto. Focus on evidence-based case management and counselling for individuals with severe mental illnesses.

This position is crucial as part of the REACH intensive case management service. Your expertise will support clients in accessing resources and skills necessary for recovery. You will work collaboratively to advocate for clients while managing a diverse caseload, directly impacting community well-being.

Key Responsibilities:
• Deliver high-quality case management and supportive counselling
• Manage individual client caseload effectively
• Conduct assessments and evaluations of client needs




• Collaborate with multi-disciplinary teams on client care
• Organize and facilitate client group sessions as needed

Requirements:
• Baccalaureate in related field with diploma in Human Services
• Three years of front-line community mental health experience
• Solid understanding of diverse cultural perspectives
• Exceptional judgement and crisis problem-solving abilities
• Commitment to ongoing career development
