Key Responsibilities:
• Conduct thorough evaluations of hospitalized participants
• Coordinate inpatient care and discharge planning

Requirements:
• Unrestricted RN license in California
• Minimum 3 years experience with older adults
• Current CPR/BLS certification necessary
• Valid driver’s license required
