Care Manager

Reporting to the Director of INTEGRAcare East, the Care Manager is mainly responsible for providing primary care management, nursing support, coordinating & scheduling care for clients to ensure they maintain their dignity, independence, and joyful living.

Responsibilities

- Manage your own INTEGRAcare clients, establishing and modifying dynamic care plans and schedules.
- Manage care teams by planning and scheduling caregivers with a focus on continuity and consistency of care.
- Help teach the new hire program and assist in the orientation process.
- Be part of the on‑call rotation.
- Participate in company and team building events.

Qualifications

- Valid CNO registration in good standing.
- 1-2 years of nursing experience (preferred).
- Bilingual in English and French.




- Great attitude and willingness to work in a fast‑paced environment.
- Robust attention to detail and desire to close the loop.
- Good written and verbal communication skills.
- Passionate about providing clients and caregivers with the highest quality care and support.
- Open to new learning opportunities, coachable and dependable.
- Valid driver’s license in good standing.

Note – this role will involve in‑person paid training as per a mutually agreed schedule at our Toronto office at 396 Moore Avenue.
