The role blends hands-on clinical practice with quality improvement, education, and clinic operations within a growing specialty infusion environment.

Youll play a meaningful role in shaping care delivery while working alongside a collaborative, multidisciplinary team.

Position Description The Nurse Practitioner will provide direct patient care within scope, supporting patients across episodic primary care, preventative care, chronic disease management, and specialty infusion services.

The role includes close collaboration with specialist physicians, particularly in gastroenterology and other specialty areas, as well as support for patients before and after biologic infusions.

Beyond clinical care, this position contributes to clinic operations and quality initiatives, including mentoring nursing staff, supporting protected infusion practices, developing policies and clinical pathways, and participating in audits and performance improvement activities.





The NP will also support patient and staff education and help build a community of practice focused on evidence-based care and continuous improvement.

Experience Minimum of 4 years of experience practicing as a Nurse Practitioner in a primary care or adult setting Experience working with complex or chronic patient populations Exposure to specialty clinics, infusion therapy, or IBD management is considered an asset Experience with quality improvement initiatives, clinic operations, or mentoring is an asset Qualifications Current registration and good standing with the College of Nurses of Ontario as an NP (PHC or Adult) Authorized to practice with appropriate Professional Liability Protection Strong clinical judgment, organizational skills, and attention to detail Excellent communication and collaboration skills Comfortable using EMRs and digital tools Adaptable, proactive, and motivated to contribute to program growth Education Masters degree in Nursing with Nurse Practitioner certification (Primary Health Care or Adult stream).
