You'll be responsible for creating personalized care plans, implementing the Newcastle Model Formulation, and directly impacting the lives of those with dementia. Key Responsibilities:
• Provide dementia-focused education and training for staff
• Create person-centered care plans for dementia patients




• Collaborate with multidisciplinary teams to enhance care
• Ensure travel to various sites for effective service delivery

Requirements:
• Valid Registered Mental Health Nurse status
• Experience in delivering relevant training across professions
• Completed Dementia Skilled Leadership Programme
• Ability to communicate effectively in clinical settings
• Membership in the Protection of Vulnerable Groups Scheme
