Key Responsibilities:
• Oversee care coordination for patients with complex wound needs
• Conduct expert assessments and collaborate on care plans




• Develop educational materials for patients and families
• Lead quality improvement initiatives and evaluate outcomes
• Support research initiatives and maintain clinical documentation

Requirements:
• Master's degree in Nursing preferred
• 5 years of relevant experience in acute care
• 2 years in wound, ostomy, and continence care
• Registered with the College of Nurses of Ontario
• Robust communication and organizational skills
