As a part of the Seniors Mental Health Behavioral Support Services team, you will support healthcare practitioners across various settings. You will share your expertise in seniors' mental health assessment and intervention, helping to cultivate effective care strategies. This integral role also involves building knowledge networks and facilitating educational programs within the community.

Key Responsibilities:



• Provide case-based consultations to healthcare providers • Develop partnerships with community healthcare organizations • Identify community learning and development needs • Participate as part of the SMH BSS team • Support system implementation for improved person-centred care

Requirements: • Degree in a Health Care Discipline, Masters preferred • Minimum of three years in community healthcare • Expertise in seniors mental and behavioral health • Knowledge of PIECES framework and GPA • Experience in educational interventions for healthcare staff
