Flemingdon Health Centre (FHC) is a registered charity and not-for-profit Community Health Centre serving Flemingdon Park, Fairview and Thorncliffe Park. Across three sites, FHC provides community-based health services rooted in health equity, inclusion, community engagement and the social determinants of health.

The Manager, Primary Care Transformation provides project management and implementation leadership for North York Toronto Health Partners (NYTHP), supporting the Primary Care Network (PCN) Strategy and expansion of Interprofessional Primary Care Teams (IPCT) in North York.

The role leads assigned workstreams related to primary care access, patient attachment, partner engagement, implementation planning, performance tracking and reporting. The Manager must work independently, manage competing priorities, exercise sound judgment and produce clear, meeting-ready materials.

- Lead assigned PCN/IPCT projects through planning, implementation, monitoring, reporting and follow-up.
- Maintain project workplans, action trackers, decision logs and risk/issue logs to support accountability and timely escalation.
- Prepare project materials, including briefing notes, presentations, status updates, summaries and partner-facing documents.
- Coordinate with NYTHP partners, PCN members and stakeholders to advance shared primary care priorities.

Meeting Management

- Plan and support select PCN working group meetings, including scheduling, agendas, materials and follow-up notes.
- Document decisions, owners, timelines, risks and next steps clearly and concisely.

Communication, Engagement & Reporting

- Lead assigned primary care engagement activities,



including provider outreach, relationship-building, PCN membership expansion and tracking.
- Draft and coordinate communication materials related to PCN priorities, meetings, initiatives and partner updates.
- Collect updates, maintain documentation, track deliverables and identify risks or gaps requiring follow-up.

Finance & Funding

- Support budget tracking, expense monitoring, grant applications and funding-related deliverables.
- Undergraduate degree in Health Administration, Health Sciences or a related field, plus a minimum of four years’ experience managing projects, implementation initiatives or multi-partner workstreams.
- Experience in healthcare, primary care, community health, community support services or Ontario Health Team-related work preferred.
- Solid project management skills, including workplan development, tracking, follow-through and delivery against timelines.
- Excellent written and verbal communication skills, including preparation of concise, meeting-ready materials.
- Sound judgment, adaptability and ability to identify risks, barriers, unclear accountabilities and decisions requiring escalation.
- Ability to work effectively with multiple partners and stakeholders in a complex health system environment.

- Incumbent will be expected to work some evening and/or weekend hours based on project demands.
