Become a pivotal part of IWK Health's Rheumatology Team in Halifax. This Social Worker role entails supporting youth and families facing chronic medical conditions with tailored counseling services.
Reporting to the Manager of Children’s Ambulatory Medical Care, you will provide social work services primarily in outpatient settings. Your role will include individual and family interventions, advocacy, and connecting families to vital resources. You will also support inpatient care when necessary, highlighting the importance of a holistic approach to patient well-being.
Key Responsibilities:
• Offer psychosocial support to families dealing with chronic illnesses
• Facilitate access to health care and community resources
• Engage in program support and evaluation
• Take part in on-call responsibilities and intake services
• Work collaboratively within a multidisciplinary team
Requirements:
• Master’s Degree in Social Work required
• Minimum of two years of related experience
• Familiarity with the Child and Family Services Act
• Solid communication and interpersonal skills
• Experience in pediatric settings is advantageous
