The role of Fracture Prevention Coordinator is essential in enhancing patient outcomes and care for individuals aged 50 and over with fragility fractures. This full time position involves educational outreach, coordination with healthcare teams, and facilitating access to vital osteoporosis services. You will implement the Fracture Screening and Prevention Program, ensuring continuous support for patients' post-fracture care.
Key Responsibilities:
• Identify and screen fragility fracture patients aged 50 and over
• Facilitate diagnostic testing for identified patients
• Communicate with patients' primary care providers
• Educate patients on bone health and fracture prevention
• Refer patients to osteoporosis specialists as needed
Requirements:
• Registered Practical Nurse (RPN) or Registered Nurse (RN)
• Active registration with the College of Nurses of Ontario
• Experience with Electronic Medical Records (EMR)
• Robust communication and teamwork skills
• Willingness to comply with Health Canada policies
