VHA is seeking part time Registered Practical Nurses to join our Hospital Partnership Program. Candidates will coordinate home health nursing services, assess patient needs, and integrate community health practices. Work hours include morning shifts and rotating weekends, requiring a valid Ontario driver’s license and reliable transportation.

Key Responsibilities:



• Assess and monitor patient care plans regularly • Evaluate care interventions through ongoing assessments • Ensure high-quality care and practice autonomy • Maintain accurate documentation as per nursing standards • Participate in standby/on-call rotation as required

Requirements: • Registered with the College of Nurses of Ontario (CNO) • Previous acute care or medical-surgical experience is an asset • CPR and First Aid certification required • Effective communication skills in written and verbal • Commitment to qualified growth and reliable availability
