Take on the role of Primary Care Nurse Practitioner with ConnectWell Community Health, serving Lanark and Carleton Place, Ontario. This permanent full time position emphasizes health promotion and comprehensive care. As an NP, you will provide essential primary health care services in collaboration with an interdisciplinary team.

Your focus will be on educating clients about health management and self-care, while also participating in crucial community outreach initiatives. This position requires evening availability and includes duties across two sites. Key Responsibilities:
Provide patient-centered primary health care
Collaborate effectively with a healthcare team
Promote client education and self-management strategies




Facilitate community outreach and health initiatives
Be available for on-call telephone support

Requirements:
Completion of a Primary Care Nurse Practitioner program
Master's degree in Nursing or equivalent recognized education
Current RN (EC) registration with the College of Nurses of Ontario
Two years of experience in a primary care nurse practitioner role
Valid driver's license and vehicle access
