As a key member of the interdisciplinary healthcare team at Bridges CHC, you will provide holistic primary care that meets the health needs of diverse patients. This position emphasizes the importance of trauma-informed care and social determinants of health, enabling you to encourage healthy practices and lifestyle changes among clients. You will support the Centre's mission through dedicated and compassionate service in the community.
Key Responsibilities:
• Assess and provide care to clients at various life stages
• Collaborate effectively within a health care team
• Promote disease prevention and educate clients on health practices
• Follow the College of Nurses of Ontario's Standards of Practice
• Participate in on-call after-hours service when needed
Requirements:
• Registered Nurse in Extended Class with the College of Nurses
• Nursing degree (Bachelor or Master) from an accredited university
• Minimum of 3 years’ experience in healthcare, community focus preferred
• Solid commitment to diversity, equity, and inclusion in practice
• Effective time management and conflict resolution skills
