Spectrum is looking for a agile Registered Nurse (RN) or Registered Practical Nurse (RPN) on a one-year contract to guide a multidisciplinary team in delivering quality integrated care. Focus on patient-centered goals, optimizing service delivery through collaboration and data-driven insights, ultimately shaping the future of healthcare in Ontario.
Key Responsibilities:
• Direct integrated palliative care team operations
• Collaborate with care teams to establish comprehensive care plans
• Monitor and oversee indirect care coordination practices
• Provide high-level support for symptom management and care advocacy
• Build strong stakeholder relationships to improve care quality
Requirements:
• RN/RPN in valuable standing with the College of Nurses of Ontario
• Minimum 5 years nursing experience, with 2+ years in management
• Experience required in palliative care settings
• Expertise in program development and evaluation
• Must possess a valid G driver's license and be available for on-call
