This is an exciting opportunity for an Occupational Therapist to join a busy, dynamic and evolving Crisis and Assessment Team in Jersey. The post holder will be a key member of a 24 hour service providing high quality, responsive mental health assessments and crisis interventions to individuals experiencing acute mental health difficulties across the island.
The role will focus on promoting a holistic, recovery focused approach through functional assessment, occupational formulation and evidence based intervention, ensuring that the unique contribution of Occupational Therapy is fully embedded within crisis and assessment pathways.
The post holder will take an active role in undertaking mental health assessments within the scope of their Occupational Therapy practice. This includes participating in urgent and planned assessments as part of the 24hour Crisis and Assessment service, contributing to risk formulation, crisis planning, and safe, person centred decision making.
The role involves working with individuals and their families who may present with a wide range of mental health conditions, often in complex and highly emotive circumstances.
As part of the MDT, the post holder will act as a specialist Occupational Therapy resource for colleagues and partner agencies, supporting integrated working and continuity of care across services. They will also contribute to service development, innovation and continuous quality improvement within the Crisis and Assessment Team.
Main duties of the job Receive, screen and scrutinise referrals to determine appropriateness in line with service criteria, identifying urgency, risk and the most suitable response.
Use excellent interpersonal and communication skills, listening to and valuing the views of others, and demonstrating respect for individuals from a wide range of social, cultural and ethnic backgrounds.
Undertake mental health assessments, including for individuals in crisis, independently within community settings, the Emergency Department and police custody. Liaise with family members, carers and professionals, taking an active role in risk assessment, formulation and safety planning, and where appropriate promoting and facilitating alternatives to hospital admission.
Liaise and work positively with members of the Crisis and Assessment Team, other departmental teams and partner organisations.
Encourage and empower service users to take an optimum level of responsibility for their individual programme of care.
Be personally responsible and professionally accountable for the management of a caseload as part of the Crisis and Assessment Team.
Produce timely, accurate and high quality written documentation and reports in accordance with statutory guidance, professional standards and Government policy.
Provide supervision, mentorship and support to students and junior members of staff within own professional scope of practice, as required.

Job responsibilities Assess, develop interventions and monitor needs for a range of highly complexconditions within a specialist area. This includes specialist evaluation, diagnosticservices, advice and training to clients, families and other professional staff on themanagement of physical and mental illness, injury or disability, prevention of deformityand deterioration, management of a client's environmental factors, vocational andeducational opportunities and leisure activities.
Undertake the role of care co-ordinator within a health and social services context,encompassing all required to assess for long term care benefit, and negotiate specialistcare packages including budgeting.
Share, interpret, implement and practice Government and National policies, standardsand guidelines within specialist area to ensure that work is conducted in accordance withorganization/service policies and within health and safety regulations.
Receive direct referrals from a range of clinical services and the general public,diagnosing and screening clients to identify how they may benefit from OT intervention,and providing subsequent comprehensive, holistic assessment using a variety ofspecialist clinical skills and individual intervention including prescription of Rehabilitationprogrammes and therapeutic activities in order to promote maximum independence ofthe client.
Independently manages cases/key working of disabled individuals and offers support tocarers to promote effective outcomes that are met consistent with the client's needs, andto refer to other agencies as required, with guidance from Senior 1 OccupationalTherapists as required.
Delivers professional evaluation, consultation, advice and training to clients, carers andrelatives, other professional staff within the hospital and community, education and thevoluntary sector as appropriate, in the use of specialised aids, assistive technology,adaptive bespoke equipment and major housing adaptations.
Maintain appropriate and timely client records of assessment,



intervention and progressto monitor effectiveness of intervention, to facilitate treatment planning and long-termclient management and to support the ongoing evaluation of the efficiency andeffectiveness of the service to inform departmental planning purposes.
Develop & deliver service specific training programmes, including supporting the trainingof Health and Social care and Occupational Therapy students and support staff. ProvideOccupational Therapy information and advice to skilled qualified and unqualifiedstaff, voluntary organisations and family carers in order to improve the understanding ofclient need and promote effective management of the client's condition to minimize theimpact on quality of life.
Act as a mentor and provide regular clinical supervision to OT staff. Advise and supporthealth and social care colleagues, trainees and support staff to ensure they aresupported to deliver their roles to the best of their abilities and to continuing professionaldevelopment.
Support the appraisal of the service's performance, identifying unmet needs,implementing the use of outcome measures to evaluate the effectiveness of the serviceand interpreting and actioning developments in Occupational Therapy practice in orderto make recommendations and contribute to organisational changes to improve theservice.
Identify necessary major adaptations required for client's homes and schools to facilitatefunction, access and mobility, working with third party providers to develop detailedbuilding specifications for the specific requirements of the clients concerned and workingclosely with clients, architects, builders and Community and Government Departments toeffect changes.
