Detailed Overview
In accordance with BCCNM standards and Fraser Health Mission, the Community Health Nurse (CHN) – Registered Nurse works independently in the community setting. The RN collaborates with an interprofessional team to manage an assigned caseload, including assessments, coaching, interventions, client care, and follow‑up. The role focuses on health promotion, chronic disease management, and coordination across the healthcare continuum.

Responsibilities

Establish therapeutic relationships with clients, ensuring autonomy and privacy.

Screen referrals, provide individualized assessments, and coordinate interprofessional care planning.

Develop comprehensive shared patient care plans in collaboration with team and clients.

Provide direct client care as needed and identify additional care services in line with guidelines.

Delegate and supervise community health workers on client‑specific tasks, ensuring training and compliance.

Facilitate care conferences, review plans, and evaluate effectiveness.





Discuss clinical findings and develop action plans with clients and families.

Advocate on behalf of clients during transitions and provide support during clinic visits.

Coordinate joint home visits and advise on community resources.

Deliver health counseling, education, and wellness promotion through clinics, phone, or home visits.

Maintain clinical records and statistical data in accordance with policies.

Engage in professional development, review literature, and attend workshops.

Participate in quality improvement, risk management, and research activities.

Orient and educate nursing staff and students, acting as a preceptor when appropriate.

Contribute to the development and revision of standards, policies, and procedures.

Perform other duties as assigned.

Qualifications
Education and Experience

Current registration as a Registered Nurse with BCCNM.

One year of recent clinical experience with complex geriatric or adult patients in an acute or community setting, including discharge planning experience.

Valid BC driver’s licence and personal vehicle access.
