Responsibilities Complete comprehensive client assessments, develop care plans, and implement and evaluate care based on best evidence plus each client’s unique needs.
Act as the point‑of‑care lead for primary provider caseloads, collaborating with other care team members and support functions.
Support clients through care transitions by connecting them with community support services.
Requirements Current Certificate of Competence with the College of Nurses of Ontario.
Availability to work days, evenings, nights, and every other weekend for 12‑hour shifts.
Updated immunization records, a clear criminal background check, and a vulnerable sector check required upon hire.
Willingness to attain required SE Health certifications.
Current CPR certificate required.
Physical demands include sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level, and various hand grips.
