Robust navigation skills and knowledge of crisis management are vital as you collaborate with providers throughout Manitoulin Island to offer tailored care.

Key Responsibilities:
• Perform client intake and suicide risk assessments
• Direct clients to appropriate Mobile Wellness staff
• Engage with agencies for case management support




• Ensure accurate and timely documentation in EMR
• Participate in regular case discussions with the team

Requirements:
• Diploma in a related social service discipline
• Minimum of three years' experience in mental health services
• Skill in crisis resolution and service navigation
• Class G driver's license, reliable transportation
• Transparent Vulnerable Sector Criminal Record Check
