Quest Community Health Centre is seeking a dedicated Family Physician in St. Catharines, Ontario, to provide exceptional care in a community-focused setting. This role emphasizes comprehensive, team-oriented care for individuals facing various barriers to health access.

As a Family Physician at Quest CHC, you will work alongside an interdisciplinary team, offering primary care that prioritizes health promotion and continuity. You will deliver services in clinics, community spaces, and through home visits, ensuring that your practice is responsive to the needs of priority populations. Your versatile schedule will include some evenings and on-call participation.

Key Responsibilities:
• Provide accessible primary health care to barriers to care
• Support and enhance sexual health and HIV care initiatives




• Deliver care across multiple settings including homes and communities
• Collaborate with teams to create effective care plans
• Engage in case conferences and quality improvement activities

Requirements:
• Licensed in Ontario with the College of Physicians and Surgeons
• CCFP certification and CMPA membership necessary
• Previous experience with priority populations
• Commitment to client-centered and trauma-informed practice
• Solid communication and team-oriented team skills
