This role operates within a Patient Model Family Health Organization and provides opportunities to work collaboratively with an interdisciplinary team. Your tasks will include managing treatment plans, educating patients, and documenting care through the Accuro EMR system. You’ll also engage in teaching experiences and community support initiatives.

Key Responsibilities:
• Deliver continuous healthcare across all ages and conditions
• Manage treatment plans for chronic and acute conditions
• Educate patients on health maintenance and prevention




• Conduct regular assessments, check-ups, and support vaccinations
• Collaborate with various healthcare staff to ensure coordinated services

Requirements:
• CFPC certification and CPSO licensure necessary
• Excellent communication and interpersonal abilities
• Ongoing commitment to medical education
• Skills in managing a variety of health issues
• Solid ethical standards and compassion for patients
