Vancouver Coastal Health is seeking a dedicated and skilled Mental Health and Substance Use Case Coordinator Outreach Nurse. In this essential role, you will collaborate with a multidisciplinary team to provide intake, triage, and outreach services for individuals with mental health issues. You will assess clients' needs and oversee their care plans.
Key Responsibilities:
• Conduct intake and outreach for mental health services
• Develop, implement, and evaluate care plans
• Administer prescribed medications and monitor effects
• Maintain detailed client records and documentation
• Actively participate in team meetings and quality initiatives
Requirements:
• Licensed as RN or RPN in British Columbia
• Diploma in Nursing or Psychiatric Nursing
• Minimum of three years relevant experience
