Provide specialized care at the Heart Function Clinic as a Registered Nurse (RN). Support patients through comprehensive assessments and clinical interventions for heart failure management.
This role, embedded in the Schulich Heart Program, emphasizes teamwork and patient-centered care. You will triage outpatient referrals, conduct nursing assessments, and perform essential tasks such as ECGs and intravenous therapy. Collaborating with the Transfusion Medicine team may also be part of your responsibilities. Maintain communication to ensure smooth onboarding for patients using the Medly Application, linking technology to clinical care.
Key Responsibilities:
• Conduct nursing assessments and record vital signs accurately
• Coordinate referral management and database updates
• Perform clinical procedures,



including iron transfusions and venipuncture
• Provide patient education and support through phone assessments
• Collaborate with the interprofessional team for holistic care
Requirements:
• Current nursing certification from the College of Nurses of Ontario
• Up-to-date BCLS certification required
• At least two years of experience in heart failure care recommended
• Proven skills in intravenous therapy and ECGs
• Solid analytical and interpersonal communication abilities
