Physiotherapist – Reactivation Care Centre (Adult) – Markham Site.

Responsibilities

- Provide assessment consultation and education to patients, family members and the interprofessional team within the acute and subacute adult patient population at Oak Valley Health – Markham site Reactivation Care Centre.
- Utilize a goal‑oriented, client‑centered approach to care.
- Participate in ongoing program development and quality improvement activities.
- Perform frequent moderate and heavy exertion: lift over 60 lbs., manage fast pace and manual patient handling including manual chest care, repositioning, transferring, lifting and ambulation activities.

Qualifications

- Current registration with the College of Physiotherapists of Ontario.
- Minimum 2 years experience in an acute care and/or subacute care setting.
- BLS certification is required.
- Previous experience with surgical, rehabilitation and emergency patient populations.
- Ability to work effectively and efficiently in a fast‑paced environment.
- Demonstrated critical thinking skills in a subacute care environment.




- Excellent discharge planning skills with knowledge of community resources.
- Excellent communication and interpersonal skills.
- Ability to collaborate with members of the interprofessional team, including support staff.
- Valuable computer skills and experience with electronic documentation preferred.
- Evidence of continuing education and ongoing professional development.
- Demonstrated good attendance and performance records.
- Commitment to a healthy and safe workplace and compliance with hospital policies, health and safety legislation and best practices.

Work Schedule

Variable schedule with a focus on Monday/Tuesday, some weekly variation and one weekend included. Shifts 10am–6pm, subject to operational changes.

Compensation

OPCOM12 range: $41.36 – $57.27 per hour.
