Bayshore HealthCare is seeking empathetic Social Workers to provide person-centered care in community settings. You will adapt social work principles to help adult clients improve their emotional and cognitive well-being. Your role emphasizes collaboration with clients, families, and healthcare partners, ensuring effective psychosocial support is delivered.

Key Responsibilities:
• Communicate with clients and professionals to coordinate services
• Maintain a manageable caseload and ensure timely assessments
• Identify and monitor psychosocial issues and treatment goals




• Advise clients on community resources and support
• Facilitate referrals to necessary services within the community

Requirements:
• Active member of Ontario College of Social Workers
• Minimum two years of community-based therapy experience
• Strong English communication skills, French is a plus
• Valid driver’s license and access to a vehicle
• Current CPR certification
