Key Responsibilities:
• Conduct initial and ongoing client assessments
• Develop and implement patient care plans

Requirements:
• Registered with B.C. College of Nurses and Midwives
• Three years of recent primary care nursing experience
• Valid B.C. Driver's License
• Knowledge of chronic disease management
