Become a vital part of the Government of Yukon as a full-time Transition Liaison Coordinator based in Whitehorse. This role focuses on coordinating services for clients with complex health needs in home care settings.

In the Health & Social Services department, you'll contribute to personalized care that enhances client independence and quality of life. This position involves screening, intake, and coordinating advanced nursing assessments by leveraging your strong interpersonal skills. Collaboration with various programs to meet complex client needs is critical to your success.

Key Responsibilities:
• Coordinate client services during transitions to home care
• Perform advanced nursing assessments as Registered Nurse
• Screen and intake clients with complex health conditions




• Build relationships with multidisciplinary teams and clients
• Ensure compliance with CPR and other required certifications

Requirements:
• Experience in home care or long-term care settings
• Current registration with Yukon Registered Nurses Association
• Strong knowledge of available health resources
• Excellent organization and communication skills
• Ability to lift and manage client transfers safely
