In this role, you will conduct client intake assessments and evaluate activities of daily living. Candidates must be prepared to assess mobility needs and complete home safety evaluations while maintaining professional documentation. You will operate in various communities based on your availability.
Key Responsibilities:
• Conduct client intakes in community and home settings
• Assess clients’ daily living activities
• Complete environmental safety assessments
• Evaluate mobility and transfer requirements
• Provide professional recommendations and documentation
Requirements:
• Registered Occupational Therapist with COTO
• At least 1 year experience in client assessments
• Experience with mobility aids and safe patient transfers
• Solid communication and documentation skills
• Reliable transportation is preferred
