Contribute to enhancing seniors' lives as a part-time Occupational Therapist with Senior Persons Living Connected (SPLC). This on-site role involves assessment and innovative support for older adults.
As a valued member of the GAIN Community Team, the Occupational Therapist will carry out Comprehensive Geriatric Assessments and provide interventions that improve daily activities. You will collaborate with an inter-professional team and engage with seniors to develop personalized care plans that enhance their well-being.
Key Responsibilities:
• Participate in Comprehensive Geriatric Assessments
• Conduct clinical triage for current referrals
• Gather and record data across assessment domains
• Provide clinical care to maintain daily living activities
• Analyze seniors' capabilities through assessments
Requirements:
• University Degree in Occupational Therapy
• Registered with the College of Occupational Therapists of Ontario
• 2-3 years experience in the field
• Knowledge of geriatric populations
• Valid driver's license for home visits
