Social Service Worker (Full time, Ongoing Contract)
The Thames Valley Family Health Team is one of the largest family health teams in Ontario. With more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive collaborative primary health care to over 165,000 patients. The Family Health Team staff includes nurse practitioners, registered nurses, social workers, pharmacists, dietitians, occupational therapists, and other health care professionals who will work with physicians as an interdisciplinary team, each using their experience and skills to provide patient centred care. The Social Service Worker will help support patients to manage personal and social challenges, offering short-term counselling, facilitating group programming and connecting individuals with community resources. In collaboration with the Interprofessional Healthcare Team, the Social Service Worker participates in the assessment, treatment and evaluation of individual, interpersonal and societal problems using a social service framework.

Requirements:

Graduate of an accredited college with a diploma in Social Service Work





Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)

Clinical experience as a Social Service Worker in a primary care setting preferred

Must be available to work evening hours

Proficient in Rapid Assessment models

Knowledge and experience in mental health assessment and management

Skills/Abilities:

Excellent clinical skills and experience assessing and managing mental health presentations and social needs commonly seen in primary care

Understanding of social determinants of health and their impact on health and well-being

Knowledge of financial, housing & transportation resources within the community

System navigation and advocacy

Ability to provide needs assessment, identification of goals that promote wellness, empowerment and autonomy

Monitor and evaluate service delivery for individual clients, communities and populations by use of mental health and addictions measurement tools and technology

Experience co-facilitating skills-based groups

Knowledge of relevant health care legislation and community mental health & addiction resources

Strong leadership, organization,



time management and interpersonal skills

Demonstrated ability to work independently and collaboratively as a member of an interdisciplinary team.

Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops, and conferences.

Experience in the use of an EMR, Microsoft Word, Excel, and Outlook

Demonstrated ability to be open and non-judgmental.

Effective interpersonal and communication skills (written/verbal)

Must have reliable transportation to allow you to travel to other locations to complete your work if needed

Hourly Rate: $26.3754 - $32.1651 (dependent on experience)
