In this critical role, you will be an integral member of the Dual Diagnosis Consultation Outreach Team, providing targeted support for individuals with dual diagnoses. You will work collaboratively with community partners to implement comprehensive treatment plans, conduct assessments, and provide crucial consultations. Your contributions will help clients develop life-enhancing skills and embrace meaningful community participation.
Key Responsibilities:
• Conduct assessments and provide clinical consultations for dual diagnosis cases
• Evaluate treatment plans in consultation with stakeholders
• Analyze occupational performance and recommend appropriate interventions
• Help structure social and leisure activities for clients
• Provide tailored education to caregivers and care providers
Requirements:
• Master’s Degree in Occupational Therapy
• Unrestricted registration with the College of Occupational Therapists of Ontario
• At least three years of relevant experience preferred
• Experience with dual diagnosis clinical cases
• Proficiency in community engagement and case management
