In this replacement role at Extendicare, you will conduct scheduled visits as a community nurse, working with a diversity of patients in their homes. Your autonomy and independent practice will allow you to establish therapeutic relationships while using appropriate assessment techniques. There are two shifts available: a temporary morning contract and a permanent evening role, both including weekends.

Key Responsibilities:
• Assess patients' needs using tailored evaluation techniques
• Monitor and execute personalized care plans diligently




• Build rapport and trust with patients through collaboration
• Maintain and update detailed patient records regularly
• Participate in required on-call coverage as needed

Requirements:
• Registered Nurse designation with CNO in good standing
• Valid driver’s license and vehicle access mandatory
• Experience in community care is highly valued
• Reliable, versatile nature and travel willing
• Passionate about enhancing community health outcomes
