This full time, 12-month contract role offers the chance to engage directly with patients who have experienced a fragility fracture. Collaborate with multidisciplinary teams to provide comprehensive screening and facilitate necessary post-fracture care. This position is vital for effectively implementing the Fracture Screening and Prevention Program and advancing community education.
Key Responsibilities:
• Screen patients aged 50+ for fragility fractures
• Facilitate essential diagnostic testing for patients
• Liaise with primary care providers regarding osteoporosis
• Connect patients with osteoporosis specialists
• Educate on bone health and prevention methods
Requirements:
• Current registration as an RPN or RN in Ontario
• Familiarity with Electronic Medical Records (EMR)
• Strong communication skills for patient interaction
• Team-oriented with independent work capability
• Fluency in a second language is an asset
