Empower refugees as a Case Worker at Polycultural, focusing on support services that facilitate effective community integration in Canada. This role includes essential case management and continuous client support.

You will engage with government-assisted refugees in a profound capacity, implementing the Client Support Services program and addressing immediate and long-term settlement needs. The role centers on service planning, facilitating referrals, and maintaining robust partnerships within the community for accessible support.

Key Responsibilities:
• Deliver full range case management for referred refugee families
• Conduct tailored needs assessments for each family member
• Plan and coordinate services to achieve client-set goals




• Foster relationships with community partners and local services
• Achieve program targets through effective service tracking

Requirements:
• Degree or diploma in Social Work or Counselling required
• At least three years’ experience in settlement services
• Registered Social Worker (RSW) is a plus
• Fluency in French needed
• Strong outreach and community development skills
