Key Responsibilities:
• Plan interdisciplinary care conferences for recent admissions
• Implement social work services to support residents




• Counsel and advocate for residents and their families

Requirements:
• Bachelor or Masters in Social Work or related field
• Active registration with the Ontario College of Social Workers
