Key Responsibilities:
• Conducting comprehensive assessments for home care clients
• Coordinating services and referrals to needed agencies
• Engaging clients and families in care planning
• Ensuring compliance with HIPA and confidentiality
• Collaborating with multidisciplinary teams

Requirements:
• Health or social science related degree
• Excellent verbal and written communication
• Strong computer and critical thinking skills
• Must possess a valid driver's license
• Experience in palliative care is preferred
