As a Clinical Pharmacist with Mackenzie Health, contribute significantly to patient care by providing expert medication management and support. You'll work within a multidisciplinary team to monitor drug therapies and educate patients about their medication needs.

Your expertise will guide the effective assessment and planning of drug therapies. By applying a pharmaceutical care model, you'll identify and prevent drug-related problems while conducting drug utilization reviews for cost efficiency. Supervision of Pharmacy students and Technicians will be a key aspect of this role.

Key Responsibilities:
• Plan and evaluate individualized drug therapy
• Provide patient education to reduce compliance issues




• Conduct drug utilization reviews for treatment efficiency
• Coordinate medication supply for continuous care
• Uphold compliance with safety policies in the workplace

Requirements:
• Bachelor Degree in Pharmacy mandatory
• Licensed with Ontario College of Pharmacists
• Previous experience in a hospital setting
• Strong empathy and communication skills needed
• Adaptable to work across Mackenzie Health locations
