At LOFT, the Behavioural Support Specialist works onsite at North York General Hospital's Reactivation Care Centre. This role requires strong clinical skills in assessment and treatment recommendations for seniors with dementia and mental health issues. Your expertise will guide the development of tailored Behavioural Support Plans, incorporating effective behavioural approaches and psychosocial support for both clients and their caregivers.
Key Responsibilities:
• Conduct comprehensive clinical assessments for clients
• Lead development and evaluation of Behavioural Support Plans




• Provide crisis intervention and urgent care planning
• Collaborate with multidisciplinary teams for coordinated care
• Deliver training and ongoing support to staff and caregivers
Requirements:
• Degree in Nursing, Social Work, or related discipline
