This position allows for hybrid patient care—consulting in person or virtually, fostering adaptability in your practice. Suitable candidates should be ready to engage with an existing patient base or work towards building one.

Key Responsibilities:
• Deliver high-quality, patient-centered medical care
• Achieve a minimum roster of 650 patients
• Collaborate within a supportive healthcare team




• Participate in after-hours patient coverage
• Maximize use of the MyCare app for care coordination

Requirements:
• Must hold an Independent Practice Certificate from CPSO
• Obtain medical liability insurance (CMPA or equivalent)
• Strong commitment to collaborative healthcare delivery
• Willing to operate under a hybrid model
