ICBC Home Care Case ManagerAbout the Role

We are seeking a compassionate, organized, and client-focused ICBC Home Care Case Manager to coordinate and oversee home care services for individuals recovering from motor vehicle accident injuries under ICBC-funded programs. The Case Manager plays a critical role in assessing client needs, developing care plans, coordinating services, and supporting clients throughout their recovery journey to promote independence, safety, and quality of life.

Key Responsibilities

- Conduct comprehensive client assessments to identify functional abilities, care needs, recovery goals, and support requirements.
- Develop, implement, and monitor individualized care plans in accordance with ICBC guidelines and client goals.
- Coordinate home care services, community resources, rehabilitation supports, and healthcare providers.
- Collaborate with ICBC representatives, families, caregivers, physicians, occupational therapists, physiotherapists, and other stakeholders.
- Monitor client progress and adjust care plans as recovery needs change.
- Advocate for clients to ensure timely access to appropriate services and supports.
- Maintain accurate, timely, and confidential documentation in compliance with ICBC requirements, organizational policies, and applicable regulations.
- Conduct regular follow-ups, home visits, and service reviews to ensure quality outcomes.
- Participate in interdisciplinary team meetings, case conferences, training sessions, and quality improvement initiatives.
- Ensure service delivery meets client safety, quality, and satisfaction standards.

Qualifications

- Bachelor's degree in Social Work, Nursing, Human Services, Rehabilitation Sciences, Psychology, or a related field, or an equivalent combination of education and experience.
- Minimum 2–3 years of case management, home care coordination, rehabilitation, or healthcare services experience.
- Experience working with clients recovering from injuries,



disabilities, or complex health conditions.
- Knowledge of ICBC-funded care services, rehabilitation programs, and community resources in British Columbia.
- Strong assessment, care planning, communication, and problem-solving skills.
- Ability to manage multiple cases and prioritize effectively in a fast-paced environment.
- Proficiency with electronic health records and case management systems.
- Valid BC Driver’s License and reliable transportation (required).

Preferred Skills

- Experience working with ICBC claims and recovery programs.
- Knowledge of home care, rehabilitation, and community support services throughout British Columbia.
- Understanding of injury management, return-to-function planning, and client-centered care practices.
- Strong documentation, reporting, and service coordination skills.
- Experience collaborating with allied health professionals and insurance-funded care programs.
- Familiarity with provincial healthcare resources and publicly funded support programs.

Job Types: Full-time, Permanent

Pay: From $30.00 per hour
