We are looking for a compassionate, organized, and motivated Client Care Manager to join our growing team. Key Responsibilities

- Conduct client assessments and develop individualized care plans.
- Establish, monitor, and update client care plans to ensure services continue to meet each client's needs.
- Maintain accurate, complete, and organized client documentation and progress notes.
- Ensure all documentation complies with company policies and applicable standards.
- Communicate regularly with clients, families, physicians, and other healthcare professionals regarding care needs and service updates.
- Build trusting relationships with clients and their families by providing ongoing support and responsive communication.
- Coordinate client onboarding, ensuring all service agreements and required documentation are completed before care begins.
- Monitor client satisfaction and proactively address concerns or changes in care.




- Collaborate closely with our scheduling and caregiving teams to ensure continuity and quality of care.
- Participate in case reviews and contribute to continuous quality improvement initiatives.

Qualifications

- Registered Practical Nurse (RPN) or Registered Nurse (RN) is preferred.
- College diploma or university degree in healthcare or a related field is considered an asset.
- Minimum of 2–3 years of relevant healthcare or care management experience.
- Strong understanding of home and community healthcare services.
- Excellent verbal and written communication skills.
- Exceptional organizational skills with strong attention to detail.
- Ability to multitask, prioritize, and solve problems independently in a fast-paced environment.
- Proficient with computers and electronic documentation systems.
- Valid driver's license and reliable vehicle.

Availability

- Full-time position
- Flexibility to work occasional evenings and weekends as required.
- Willingness to provide telephone consultation and support for urgent situations when off duty.

Pay: $55,000.00-$80,000.00 per year
