The Dufferin Area Family Health Team (DAFHT) is seeking a compassionate, skilled, and community-focused Mental Health Community Case Manager to join our interdisciplinary team serving the Dufferin-Caledon region.

About the Role

As a Mental Health Community Case Manager , you will play a critical role in supporting individuals experiencing mental health challenges and the social determinants that impact overall health and quality of life. Working closely with primary care providers, mental health professionals, and community partners, you will provide person-centered case management that empowers individuals to access resources, build resilience, and achieve their goals.

This position is ideal for a social worker who enjoys working both independently and collaboratively, is knowledgeable about community resources, and is committed to delivering equitable, compassionate care.

What You Will Do

As a valued member of our interdisciplinary team, you will:
Provide comprehensive community-based case management for adults experiencing mental health concerns, addictions, developmental challenges, complex health conditions, and social vulnerabilities.

Conduct strength-based assessments to identify client needs, goals, risks, barriers, and available supports.

Partner with individuals to develop personalized care plans and actionable goals that promote recovery, independence, and overall well-being.

Support individuals experiencing challenges related to housing instability, financial hardship, employment, disability, social isolation, chronic illness, and other psychosocial stressors.

Assist clients in navigating healthcare, social service, housing, legal, and community systems.

Facilitate referrals and connections to appropriate community resources and support services throughout Dufferin-Caledon and surrounding areas.





Provide guidance and support with applications for programs such as Ontario Works, Ontario Disability Support Program (ODSP), Canada Pension Plan Disability (CPP-D), and other financial assistance programs.

Complete risk assessments and provide crisis support, safety planning, and appropriate follow-up.

Advocate for equitable access to services and supports while promoting client self-advocacy whenever possible.

Collaborate with physicians, nurse practitioners, psychiatrists, therapists, and community partners to coordinate care and improve client outcomes.

Maintain accurate clinical documentation and meet professional, organizational, and regulatory standards.

Demonstrate cultural humility, inclusivity, and sensitivity toward diverse populations, including differences in culture, race, religion, age, gender identity, sexual orientation, ability, and socioeconomic status.

What You Bring

Required Qualifications

Bachelor of Social Work (BSW) from an accredited institution.

Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).

Minimum three (3) years of experience providing case management services to adults with mental health concerns, addictions, developmental disabilities, complex health needs, or related populations.

Demonstrated experience conducting assessments, care planning, crisis intervention, and risk management.

Thorough understanding of the social determinants of health and their impact on mental wellness.

Strong knowledge of community programs, mental health services, and social support resources within Dufferin-Caledon and surrounding regions.

Excellent interpersonal, communication, advocacy, and problem-solving skills.

Proven ability to work independently while contributing effectively within an interdisciplinary team environment.

Proficiency with electronic medical records and clinical documentation.

Valid Ontario Driver’s License, access to a reliable vehicle,



and appropriate automobile insurance.

Successful Criminal Record Check with Vulnerable Sector Screening.

Preferred Qualifications

Master of Social Work (MSW).

Experience working within primary care, community health, family health teams, hospital discharge planning, or community mental health sectors.

Knowledge of trauma-informed, recovery-oriented, and client-centered approaches to care.

Working Conditions

Permanent Full-Time position (37.5 hours per week).

Hybrid work environment, combining community-based, clinic-based, and virtual care.

Valid Ontario Driver’s License, reliable vehicle, and appropriate automobile insurance.

Regular travel throughout Dufferin-Caledon, including Orangeville, Shelburne, Bolton, and surrounding communities.

One evening per week is required based on client/ program needs.
