Qualifications:

As a result of accreditation, this position will be required to work up to full-time hours from approximately August until December, 2026

 Current Certificate of Competence from the College of Nurses of Ontario as a Registered Nurse required
 Master’s Degree in Nursing preferred

Principal Duties:





 Provides clinical nursing specialty to support and coordinate a consistent stroke care team across the continuum
 Promotes the implementation and uptake of stroke best practice recommendations across the continuum
 Follows patients and families across the continuum of care, acting as a point of contact, completing required assessments, providing education, navigation and support across transitions
 Responsible for the coordination of comprehensive care the Secondary Stroke Prevention Clinic for individuals at risk for cardiovascular disease and stroke or who have had a stroke/TIA, using a case management approach, in an interdisciplinary team environment in collaboration with designated internal medicine physicians
 Triages referrals for the Secondary Stroke Prevention Clinic according to priority
 Provides treatment, follow-up care and evaluates outcomes at scheduled intervals for designated patients
 Develops and implements standardized patient / family education tools
 Provides cardiovascular and stroke care education to patients, caregivers as well as develops and supports health care provider education including best practices and stroke protocols and procedures
 Establishes strong linkages with stroke care partners across the continuum




 Critically analyzes best practice evidence and incorporates appropriate interventions into clinical practice
 Completes routine data collection, interpretation and reporting
 Participates in unit/department/hospital/community Quality Improvement Initiatives across the continuum of care
 Participates on various organizational and regional committees, working groups and forums
 Completes continuing education requirements as necessary
 Champions and participates in activities to sustain the standards and protocols of Accreditation Canada’s Stroke Distinction
 Other related duties as assigned
