Programme Coordinator (Geriatric Medicine)

We are looking for a compassionate, proactive, and organised individual to support the delivery of comprehensive geriatric assessment services. In this role, you will work closely with Geriatricians, nurses, allied health professionals, patients, caregivers, and community partners to ensure holistic and coordinated care for older adults.

Responsibilities

Geriatric Assessment

Conduct outpatient geriatric cognitive assessments in accordance with established clinical protocols.

Gather and document relevant patient information to support clinical assessment and care planning.

Provide patients and caregivers with information on cognitive health and available support services.

Care Coordination

Conduct pre-appointment calls to patients and caregivers to facilitate completion of assessment questionnaires and prepare them for appointments.

Coordinate referrals to community services, healthcare providers, and relevant agencies based on identified patient and caregiver needs.

Serve as a liaison between patients, caregivers, healthcare professionals,



and community partners to ensure continuity of care.

Organise and coordinate caregiver support groups and educational programmes.

Programme and Administrative Support

Support the planning and execution of geriatric service programmes, workshops, support groups, and outreach events.

Maintain accurate records, databases, and service documentation.

Collect, track, and compile data for reporting, audits, and quality improvement initiatives.

Provide logistical and administrative support to the geriatric multidisciplinary team.

Quality Improvement

Participate in service development and quality improvement initiatives.

Support data collection and reporting to evaluate programme outcomes and enhance service delivery.

Requirements

A Diploma in any dicipline

An experience in healthcare will be beneficial

Bilingual capabilities would be an advantage to facilitate communication with patients and caregivers
