Join Southeast Resource Development Council as a Jordan’s Principle Off Reserve Case Coordinator. This temporary role supports First Nations children in navigating public services through direct case management.
Reporting to the Children and Youth Program Manager, you will assist families in accessing essential services, ensuring a smooth process under Jordan’s Principle. Your work will involve conducting assessments, promoting the program, and facilitating community partnerships. We are seeking candidates with solid communication skills and a passion for Indigenous health.
Key Responsibilities:
• Facilitate program delivery and workshops
• Execute client intake and develop case plans
• Create partnerships with community service providers
• Maintain accurate documentation and reports
• Conduct follow-ups to monitor client progress
Requirements:
• Bachelor’s degree in social work, nursing, or related field
• 3-5 years of relevant experience
• Knowledge of case management and Jordan’s Principle
• Valid driver’s license and vehicle access
• Strong interpersonal and communication skills
