Your role as a Field Case Manager will involve collaborating with healthcare providers and patients to ensure seamless medication management. Working primarily from home, you will also establish in-person connections to provide personalized support. This position requires solid organizational capabilities and excellent communication skills to facilitate program success.
Key Responsibilities:
• Engage as primary contact for local physicians
• Foster relationships with patients and program partners




• Train healthcare staff on program services and resources
• Ensure timely, accurate patient documentation
• Tailor program support according to provider needs
Requirements:
• RN or a relevant degree in pharmacy or science
• Previous experience in case management is desirable
• Familiarity with public or private reimbursement
• Bilingualism required for Quebec positions
• Ability to effectively set up a home office space
