In this role, you will supervise the day-to-day operations of the Home At Last Program, ensuring timely and secure transitions from hospitals to homes. Coordination with hospitals and community partners is essential, alongside managing staff and resources. Strong communication and relationship-building skills are critical for success.
Key Responsibilities:
• Manage Home at Last Program operations effectively
• Coordinate scheduling and staffing resources
• Build collaborative relationships with care providers
• Support quality improvement initiatives across the program
• Ensure adherence to organizational policies and regulations
Requirements:
• Education in health care or social services field
• At least 3 years in a supervisory role
• Excellent leadership and communication abilities
• Valid Ontario Driver's Licence and access to a vehicle
• Proven ability to manage priorities effectively
